Registration & Policies
How to Register
When you are interested in registering at Premiere Dance Academy, the first step is to contact us. We only accept registrations by telephone, in person at our studio, or by fax, and we can provide information on schedules and availability in our programs before starting the registration process with you.
Once you have contacted the studio to check availability and start the registration process, if you are registering in person or by fax you may download, print and fill out the appropriate form(s) below to save time.
We accept Visa, MasterCard, cash, cheque, money order or bank draft.
Registration Forms
You must first contact the studio before completing these forms. The may then be downloaded and filled out if you are registering in person or by fax.
Program Policies
For information on some of the policies that we follow for our programs, please choose a program type below.

Dance Study Program Policies
The Dance Study Program has a minimum of 6+ hours of training per week, plus other prerequisites.
- Registration is in person, by phone in, or by fax.
- Phone in or Fax: Payment is taken by Credit Card OR Cheques/Cash must be dropped off at the office within 48 hours. If payment is not received, registration will be void without notification.
- Payment is by cash, cheque, credit card (Visa or Master Card), or Money Order/Bank Draft.
- Monthly payment: For enrolment in the Dance Study Program, the following payment schedule is in effect:
- A non-refundable registration fee is required.
- Monthly cheques dated the 1st of each month (September - May) are required upon registration day, or a credit card imprint with a signed authorization for monthly withdrawals.
- June payment is included with September, split into September and October payments OR Pre-Paid in August.
- NSF cheques or declined credit cards payments are subject to a service charge of $25.00
- Transfer requests will be taken as long as there is availability.
- Missed Classes
- There will be NO refund for missed classes by a student
- If a teacher cannot be available for any class, a sub is brought in or the instructor will make arrangements for a make-up class on a future date.
- Classes during competition may be altered.
- Student Withdrawal / Non-Medical Cancellation
- Cancellations are only accepted up until November 1st (one moth prior to the December payment).
- Once a payment has been deposited a cash refund cannot/will not be issued. A program voucher will be issued minus an administration fee only up until the November 1st deadline.
- After December 1st your dancer may cancel but all fees for the entire season will be processed. NO EXCEPTIONS.
- After December 1st program vouchers will be issued for medical cancellations only.
- The Registration Fee and June payment is non-refundable at anytime
- Changes to classes are subject to fees remaining as registered. NO exceptions – Thank You
- Changes / reduction in fees will be in effect 1 month AFTER your notification.
- If you are adding classes, added class fees are effective immediately.
- Program vouchers have no cash value, can only be used at Premiere Dance Academy, and are valid for 12 months from the issue date. No exceptions.
- Medical cancellation will be accepted with a doctor's note (faxed in or dropped off to the office only). A program voucher will be issued minus an administration fee for a prorated amount for classes already taken place.
- Premiere Dance Academy has the right to accept, refuse or revoke any application or registration. The decision is at the sole discretion of the Artistic Director / Owner of the Academy.

10 Week Program Policies
- Registration

- Registration is not deemed complete until the Registration Form has been signed and payment received.
- Phone in or Fax: Payment is taken by Credit Card OR Cheques/Cash must be dropped off at the office within 48 hours. If payment is not received, registration becomes void without notification.
- In Person: Payment is due at time of registration
- Payment is by cash, cheque, credit card (Visa or Master Card), or Money Order/Bank Draft
- Transfer requests will be taken as long as there is availability.
- Missed Classes
- There will be NO refund for missed classes by a student.
- If a teacher cannot be available for any class, a sub will be brought in.
- Student Withdrawal / Non-Medical Cancellation
- Ten week programs are NON-refundable. No exceptions.
- Program vouchers have no cash value, can only be used at Premiere Dance Academy, and are valid for 12 months from the date issued. No exceptions.
- Medical Cancellation will be accepted with a doctor's note (faxed in or dropped off only). A program voucher will be issued minus an administration fee for a prorated amount for classes that have already taken place. If 5 or more classes have been completed no voucher will be provided.
- Premiere Dance Academy reserves the right to cancel courses. In this case a refund will be issued. (Amount will be prorated if classes have already taken place)
- All NSF cheques and declined credit card payments are subject to a service charge of $25.00
- Premiere Dance Academy has the right to accept, refuse or decline any applications for registration to the Academy at any time. This decision is at the sole discretion of the Artistic Director. The Artistic Director will review registrations and you will be contacted only if there are any concerns.

- Trimester installation payments are required for August 1st, December 1st, and March 1st.
- If the 1st falls on the weekend or a holiday, payment will be taken the business day prior.
- Family Rates
- If you have two or more children enrolled, you pay only the Family Rates for each class taken.
- If one of your children withdraws/cancels your fees will be changed to the regular rate
- Transfer requests will be taken as long as there is availability.
- Missed Classes
- There will be NO refund for missed classes by a student.
- If a teacher cannot be available for any class, a sub is brought in or the instructor will make arrangements for a make-up class on a future date (which is not a common practice).
- Student Withdrawal / Non-Medical Cancellation
- Cancellations are ONLY accepted up until November 1st (one moth prior to the December trimester payment).
- Once a payment (August 1st, December 1st and/or March 1st) has been deposited a cash refund cannot/will not be issued. A program voucher will be issued minus an administration fee only up until the November 1st deadline.
